I am running into a similar issue with a fresh install of HexOS. Setting the app pool and restarting failed to fix my issue so I looked in the logs for the active tasks. The error I was seeing was that my version of TrueNAS-SCALE-24.10.0.0 was below the required TrueNAS-SCALE-24.10.2.2 version for Immich.
In the TrueNAS-Scale UI, I updated to 24.10.2.2 with no issues (HexOS is still working as expected) but now I am seeing the following error when attempting the install:
[EFAULT] Failed 'up' action for 'immich' app. Please check /var/log/app_lifecycle.log for more details
